No Slip Waiver and Release

By signing this Waiver and Release form I authorize the Biscuits & Bath staff to walk my dog with only his/her own collar or harness and NOT a Biscuits & Bath required slip collar. I understand that not using a slip collar to walk my dog is against Biscuits & Bath’s safety policy and presents a significant additional risk to my dog’s safety up to and including death. I certify that my dog has a legitimate medical reason to not be walked with a slip collar as described below. I will in no way hold Biscuits & Bath responsible for any negative outcome that occurs as a result of not using a slip collar to walk my dog. I shall hold Biscuits & Bath harmless from, and make no claim against and indemnify Biscuits & Bath against any costs, damages, claims or expenses that may result from injury to my dog that might result from not using a slip collar.
